Shawnessy Registry

Disabled placards are designed to assist those who are unable to walk more than 50 meters or 150 feet. In order to qualify for a temporary, long term or permanent placard you must have your physician fill in the application form which can be found here (Link to app) or you may pick up the application at our office in person. The form must be filled out in its entirety. Once it is completed the person listed on the application must be present in the registry to obtain the paperwork. They must bring Government Issued Photo Identification. If they are unable to attend the office in person, a written authorization may be given to another representative.
